Fall Leadership Training – Session 2 – October 21-22, 2025

As the Chapter Adviser, I acknowledge that by submitting this registration, I agree to full payment for events and items for which my chapter registers. I understand that payment is due immediately upon submission, and that failure to remit payment will result in my chapter being restricted from participating in future events until the outstanding balance is resolved. I also acknowledge that all registrations are final and non-refundable. Please do not proceed with registration unless you are certain of your chapter’s attendance, as payment by check is required immediately following submission.

Location: Nuhop (Main Campus) – 1077 Township Road 2916, Perrysville, Ohio 44864

Event Date: October 21-22, 2025

  • Registration 11:00 a.m. - 11:30 a.m.

Registration Fee: $225.00 per person (includes t-shirt, lodging, meals, training, materials, and supplies)

Registration closes on Monday, September 29, 2025 (125 participants maximum per session)

  • Challenge Course and Program Participation Waiver - each attendee must complete the form with parent signature if a minor - turn in at the Registration table upon arrival
  • Adult Conduct Form - each attendee must complete the form and turn in at the Registration table upon arrival
  • Student Conduct Form - each attendee must complete the form with parent signature - turn in at the Registration table upon arrival

All invoices are final once submitted. No late or on-site registration will be available.
